Meal Prep Order Form
How to use this form: Please choose 2 selections from each category Breakfast, Lunch and Dinner and choose 4 selections from the Snacks category. You will need to place your order each week in order to receive food.
Also, you will choose between Standard and Plus Portions. Standard has 4 oz protein servings and proportional sides for $150 per week. Plus has 6 oz protein servings and proportional sides.
Standard daily meals contain approximately 1600 calories and macros will be included. Plus daily meals contain approximately 2000 calories and macros will be included.
Orders for the upcoming week must be received by the Wednesday prior and will be available for pickup Sunday prior.
Payments - After your order is placed, an invoice will be sent to your e-mail to make payment via Paypal or through Venmo. Payment must be made before food preparation will begin.

You choose two breakfast options, two lunch options, two dinner options and four snack options on the form to the right. One of these options will be repeated three times for Monday, Wednesday and Friday consumption while the other will be repeated twice for Tuesday and Thursday consumption.
